Here are all the forms you will need to get your student ready for the next event!

Youth Registration:

Filling out this online form will ensure that your student has a nametag on Sunday mornings, that they will receive text messages of Scripture and reminders and that you will receive information via mail or e-mail about upcoming activities.

ZPC Parent Permission Slip 

This form is needed for all events outside ZPC, please download and bring this with you or send it to Lisa at the aove address.

Great Escape (YCM) form

To register for Great Escape you will need to turn in your $100 deposit, this form, the ZPC parent permission slip (above) and a copy of your insurance card by May 8th.  More information here.

Mexico Caravan form

To attend the Mexico Caravan to Playa Del Carmen please turn in the entire amount due, a copy of your insurance card and the above form by April 17th. More information here.
